About Community Forward SF:

Community Forward SF is a network of programs dedicated to supporting individuals experiencing homelessness and complex health and social needs. Our integrated services span housing, behavioral health, medical respite, substance use recovery, and survivor support — meeting people where they are, with services designed around their needs.

Our work is rooted in trauma-informed care and a genuine respect for the resilience of the people and communities we serve. Founded in 1978, our mission is to empower women and all people to break cycles of housing instability and violence.

San Francisco's homelessness crisis falls hardest on women — particularly women of color, transgender women, and survivors of domestic violence. Community Forward SF centers women in our care model because we know that when we get care right for those facing the greatest barriers, we make the entire system stronger and more accessible for everyone.

The Director of Mental Health Outpatient Services (DMHOP) provides clinical and operational leadership for a multi-site program delivering integrated behavioral health services within housing and community-based settings.

This is a hands-on leadership role that blends clinical supervision, program oversight, and staff development. The DMHOP supports a multidisciplinary team serving individuals with complex needs, including mental health conditions, substance use, histories of trauma, and housing instability.

Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
