Hourly Wage: $29.00/hour

Hours: Monday-Friday, 9am-5pm

We focus on the hardest to reach, most vulnerable people: Women, Medical Crisis, Substance Use Disorder, and Older Adults. Our programs meet the needs of people, rather than demanding that people meet the needs of our programs.

 

JOB SUMMARY:

Community Forward SF is hiring a Case Manager II for A Woman's Place, San Francisco's only 24-hour shelter for all women. Case Manager IIs assist our clients with all needs relating to housing, coordinating care to maintain mental and physical health as well as personal safety, and assist in navigating a complex system by providing information, education, and referral services.  

 

ESSENTIAL JOB FUNCTIONS:

  • Ensure that clients are treated with dignity and respect, and that their physical, psychological, and emotional needs are adequately met while residing at the facility.  
  • Support our mission to promote stabilization, hope, and healing for adults experiencing homelessness in San Francisco. 
  • Conduct intakes and assessments with a caseload of 25 clients, who are utilizing Department of Homelessness & Housing (HSH) beds at our A Woman’s Place shelter.
  • Assist in orienting clients to program guidelines, schedule, staff & support services.
  • Create and update Individual Service Plans (ISPs) for each client.
  • Meet with clients regularly (weekly or at minimum 1x/mo) to assist them in executing their ISP and track their progress towards achieving individual goals and objectives.
  • Facilitate 1-2 support groups, social events or recreational activities per week.
  • Collection and maintenance of client identification documents, Releases of Information (ROIs), and other housing-related documents.
  • Ensure access to resources that help stabilize clients’ through benefits advocacy, housing, health insurance, medical status, and other needs, including social service systems and other agencies.
  • Provide ongoing support services including, but not limited to, case management, counseling, crisis intervention, intake/assessment, and advocacy.
  • Assist clients with the Housing Referral Status & ensure clients are “document-ready”
  • Conduct regular consultation with Coordinated EntryHousing Navigation Staff.
  • Conduct regular coordination with the Human Services Agency (HSA) to assist clients in obtaining health insurance, food stamps, and other financial or vocational benefits.
  • Escort clients to critical off-site appointments specific to their housing acquisition.
  • Assist clients in developing external support systems.
  • Assist with medication monitoring.
  • Engage with clients about Exit Planning and Discharge Summaries.
  • Maintain client files and associated documentation in the ONE System and in physical charts, in compliance with HIPAA regulations. 
  • Composition and submission of progress notes to supervisor(s) & input into electronic health record systems on a daily and weekly basis. 
  • Coordinate care and consult with teammates, supervisor(s), and outside providers.
  • Adhere to established policies and procedures.
  • Mandatory training and meeting attendance.
  • Other duties as assigned.
 

BACKGROUND & EXPERIENCE: 

  • Bachelor’s (B.A.) degree or equivalent years of work experience.
  • Previous case management experience and/or training. 
  • Comfort with technology including knowledge of Microsoft Office Suite, Google Suite, and city housing databases, including the ONE System.
  • Strong written and oral communication skills
  • Ability to work both independently and cooperatively.
  • Ability to effectively de-escalate conflict
  • Thorough knowledge of homelessness, trauma, domestic/intimate partner violence, substance use disorder and recovery, HIV/AIDS, harm reduction, mental health disorders, neurodiversity, LGBTQIA+ communities, and systems of oppression.
  • Ability to work sensitively with individuals from diverse cultures, languages, abilities, backgrounds, and identities.
  • Ability to write and speak English effectively.
  • CPR and first aid certification required within 90 days of hire.
 

PREFERRED QUALIFICATIONS:

  • Multicultural, multilingual
  • Experience with and passion for working with at-risk client populations
  • Lived experience with homelessness and/or other systems of oppression 
  • Strong self-awareness, cultural humility, and commitment to personal praxis.
  • Good organizational skills
  • Knowledge of San Francisco community resources, including the ONE Coordinated Entry system.
 

PHYSICAL REQUIREMENTS: 

  • Ability to stand and walk as needed throughout the shift for extended periods of time. 
  • Ability to lift up to 50 lbs.
  • Ability to sit and use a computer, including keyboard and visual display terminal, for extended periods of time. 
  • Finger, hand, and wrist agility.
  • Ability to complete tasks in an environment where interruptions are the norm and background noise is present.
  • Ability to move through buildings commonly including stairs, between buildings, and out in the community.
